Abstract

Bleeding after total knee arthroplasty can result in significant morbidity and increases the need for blood transfusion. The proper use of intraoperative adjunctive topical hemostatic agents can enhance hemostasis perioperatively, potentially reducing blood transfusions. In this prospective study, 157 consecutive patients undergoing primary total knee arthroplasty received FLOSEAL (FLOSEAL Hemostatic Matrix; Baxter Healthcare Corporation, Hayward, California), a gelatin thrombin hemostatic matrix, 5 mL (74 patients) or 10 mL (83 patients). All patients received warfarin as thromboprophylaxis starting the day after surgery. Data were extracted via hospital chart review from 100 consecutive patients who underwent total knee arthroplasty and immediately preceded the FLOSEAL groups and did not receive FLOSEAL (control group). Postoperative drainage was significantly lower in the FLOSEAL 5 mL (236.9 mL) and 10 mL (120.5 mL) groups compared with the control group (430.8 mL; P<.0001 for both). The FLOSEAL 10 mL group had significantly less drainage than the FLOSEAL 5 mL group (P<.0001). The predicted probability of transfusion in the FLOSEAL 5 mL group was not significantly different compared with the control group (6.0% vs 7.6%, P=.650). The predicted probability of transfusion was lower in the FLOSEAL 10 mL group compared with the control group (0.5% vs 5.5%; P=.004). Within the FLOSEAL 10 mL group, application of FLOSEAL either before or after tourniquet release had a similarly significant effect on drainage volume and predicted probability of blood transfusion. No differences in outcomes were observed by type of anesthesia used. No adverse events occurred related to FLOSEAL use. [Orthopedics. 2015; 38(2):e118-e123.].

Hospitalization for Total Hip Replacement Among Inpatients Aged 45 and Over: United States, 2000–2010

 

Total hip replacement, in which both the head of the femur and its socket are replaced, is done to restore movement to hips damaged by osteoarthritis, late-stage degenerative bone and cartilage disease, or other injuries and disease. The number of total hip replacements is expected to increase over the next few decades.
A new NCHS report show trends and estimates of the number and rate of total hip replacements and average length of stay among inpatients aged 45 and over.
Key Findings from the Report:
  • In 2010, 310,800 total hip replacements were performed among inpatients aged 45 and over.
  • The number and rate of total hip replacements among inpatients aged 45 and over increased from 2000 to 2010: from 138,700 to 310,800 in number and from a rate of 142.2 to 257.0 per 100,000 population.
  • The age distribution of inpatients aged 45 and over who received total hip replacements changed significantly between 2000 and 2010, with the percentage of total hip replacements increasing for younger age groups and decreasing for older age groups.
  • The average length of stay after total hip replacement among inpatients aged 45 and over decreased from 2000 to 2010, from nearly 5 days to just under 4 days.

Abstract

OBJECTIVE:
To assess the safety and efficacy that compare bipolar hemiarthroplasty with unipolar hemiarthroplasty for the treatment of femoralneck fracture in the patient aged more than 65 years.

METHODS:

We searched databases including PubMed Central, MEDLINE (from 1966), EMBASE (from 1980) and the Cochrane Central Register of Controlled Trials database. Only prospective randomized controlled trials (RCTs) that compare bipolar hemiarthroplasty with unipolar hemiarthroplasty for the treatment of femoral neck fracture in the elder patient were included. RevMan 5.2 from the Cochrane Collaboration was applied to perform the meta-analysis.

RESULTS:
Six relevant RCTs with a total of 982 patients were retrieved. From this meta-analysis, mortality rates showed no statistical difference between two treatments, 14.7 % for bipolar versus 13.8 % for unipolar. The acetabular erosion rates were significantly different between two groups (P = 0.01), 1.2 % in bipolar versus 5.5 % in unipolar group. Overall complication rates, dislocation rates, infection rates and reoperation rates between two groups showed no statistical difference (P > 0.05). Neither of two treatments appeared to be superior regarding the clinical function assessed by Harris hip scores or return to pre-injury state rates (P > 0.05).
 
CONCLUSIONS:
Both bipolar and unipolar hemiarthroplasty for the treatment of elderly patient suffering displaced femoral neck fracture achieve similar and satisfy clinical outcome in short-term follow-up. Unipolar hemiarthroplasty seems to be a more cost-effectiveness option for elderly patient.

OBJETIVO:
Para evaluar la seguridad y eficacia que comparan hemiartroplastia bipolar con hemiartroplastia unipolar para el tratamiento de fractura de cuello femoral en el paciente de más de 65 años.

MÉTODOS:
Se realizaron búsquedas en bases de datos incluyendo PubMed Central, MEDLINE (desde 1966), EMBASE (desde 1980) y el Registro Cochrane Central de Ensayos Controlados base de datos. Sólo se incluyeron ensayos controlados aleatorios prospectivos (ECA) que comparen la hemiartroplastia bipolar con hemiartroplastia unipolar para el tratamiento de la fractura del cuello femoral en el paciente anciano. RevMan 5.2 de la Colaboración Cochrane se aplicó para realizar el metanálisis.

RESULTADOS:
Se recuperaron seis ECA relevantes con un total de 982 pacientes. A partir de este meta-análisis, las tasas de mortalidad no mostraron ninguna diferencia estadística entre los dos tratamientos, el 14,7% para el trastorno bipolar en comparación con 13,8% para la unipolar. Las tasas de erosión acetabular fueron significativamente diferentes entre los dos grupos (p = 0,01), el 1,2% en bipolar frente al 5,5% en el grupo unipolar. En general las tasas de complicaciones, tasas de luxación, las tasas de infección y tasas de reintervención entre dos grupos no mostraron diferencia estadística (p> 0,05). Hemiartroplastia unipolar parece ser una opción más coste-eficacia para el paciente de edad avanzada.

Abstract


After implementation of a 'fast-track' rehabilitation protocol in our hospital, mean length of hospital stay for primary total hip arthroplasty decreased from 4.6 to 2.9 nights for unselected patients. However, despite this reduction there was still a wide range across the patients' hospital duration. The purpose of this study was to identify which specific patient characteristics influence length of stay after successful implementation of a 'fast-track' rehabilitation protocol. A total of 477 patients (317 female and 160 male, mean age 71.0 years; 39.3 to 92.6, mean BMI 27.0 kg/m(2);18.8 to 45.2) who underwent primary total hip arthroplasty between 1 February 2011 and 31 January 2013, were included in this retrospective cohort study. A length of stay greater than the median was considered as an increased duration. Logistic regression analyses were performed to identify potential factors associated with increased durations. Median length of stay was two nights (interquartile range 1), and the mean length of stay 2.9 nights (1 to 75). In all, 266 patients had a length of stay ≤ two nights. Age (odds ratio (OR) 2.46; 95% confidence intervals (CI) 1.72 to 3.51; p < 0.001), living situation (alone vs living together with cohabitants, OR 2.09; 95% CI 1.33 to 3.30; p = 0.002) and approach (anterior approach vs lateral, OR 0.29; 95% CI 0.19 to 0.46; p < 0.001) (posterolateral approach vs lateral, OR 0.24; 95% CI 0.10 to 0.55; p < 0.001) were factors that were significantly associated with increased length of stay in the multivariable logistic regression model.
